As far as I know it is Ferrari 250GT by Pininfarina, but what's an exact name of this car, please? Is it a one-off?

This car was simply called Ferrari 250 GT Spider, and was presented at the '57 Geneve salon, then at the Turin show.
A similar design was shown in the 58 with and without a big air intake on the side and an higher but smaller air intake on the hood..
I took pictures of all these versions and I'll upload them.
It was a one off and the Pininfarina Catalogue calls it 250 GT Spider Speciale, but the magazines from '57 I have call it simply 250 Gran Turismo Spider

It will take some time, since there are really a lot of differen versions, by now I can tell you that the one in your pic is not the first, Collins' one, because that one had not the chrome surround on the windshield.
