I don't know if we are talking about the same car, but in QR168 p98 a Frua designed Poccardi Fiat 125 special coupé is present.

KhADI 29 (1990, VAZ 2106). Actually, the situation with KhADI Formula 1600 cars is quite complicated for me now, but I think that this is a picture of KhADI 29, although somewhere it is identified as -31.

KhADI has also built a number of other Formula 1600 cars, including Estonia 25.LPIA KhADI, KhADI-31 and KhADI-33, but I'm not sure they've had the 124 derived engines.

Russian Wikipedia mentions that in Cuba there is an almost serial production of Lada 2101 based limousines, mostly used as taxis.
Any chance to get the name of the coachbuilder? Or are them just self-made?

Here is one more car from KhADI - KhADI 15 from 1981 with VAZ 2103 engine, built to celebrate the institute's 50th anniversary. As far as I know, this was a record car, but it has not achieved any notable results.

Among the Metalex cars the ones I could find fith 124 derived engines are MTX2-01 and 2-03 sportscars (don't know about the 2-02) and some single seaters: MTX1-02, 1-03, 1-04 KIN, 1-06, 1-06B and 1-06cx.

Also the book by Michal Velebny features a number of other Lada powered Czechoslovak cars (but I feel like you may have this book, as you have listed all of these cars already).

Like I did for Formula Monza and Formula 850, I will include a paragraph only about manufacturers about at list a very few info will be found... I think it would be a neverending work to list every Formula Easter car, even if I like them very much. Furthermore, I don't speak Russian, neither Czeck nor other eastern languages and can only trust on the (very poor) Google translations.
On the other hand, it will be a much more interesting research than for the previous books because this time the wide part is really new for me!  :D

As far as I've read, "Jamnik" is not the maker, but the nickname of the stretched 125P. In Polish it means dachshund dog.
The cabriolet is an official Fiat (FSO) prototype, used for the Prime Minister in 1974 for the 30th Socialist Poland Republic anniversary. This appears in an official period FSO book, along with a limousine.
The other has been made later for professional purposes (touristic visits, hotel and business shuttle).
Only the station-wagon,  IMO is a self-made conversion, by some unknown maker.
