Looks as if it should be a transitional version of the Orient Buckboard which originally had no springs and later had semi-elliptic on the front. If so, it was made by Waltham Mfg. Co. in Waltham, Mass.
But you say it's a one-off, so who knows?

Sorry, I didn't mean to be confusing. North Central states are generally defined as Ohio, Indiana, Illinois, Wisconsin and Michigan.

I had thought that narrowing down the state possibilities might yield a find, but so far no such luck. So I will have to narrow the odds further. Is the state in question Ohio?

I've found the picture
Charles Pitkins from Lake View, Chicago 1902
The handwrite in the back explains that Charles Pitkins at the wheel. And He with Fred Norrd (not readible) built the car.
