Well I have located some bits and pieces, but I don't know if I have enough for a point.  This car was constructed in 1922 by Officine Meccaniche of Milano.  It seems to have the rather ungainly name of Achille Andreoli Codigoro Ferrara Auto.  Since I speak no Italian, I can only assume that this was a car specifically built for Achille Andreoli & Figli of Codigoro (Province of Ferrara), Italy.  I have found period advertising which leads me to believe that  Achille Andreoli & Figli may have been a dealer of motor vehicles in Codigoro, and this car was a private label product built for him by OM.  As to what is so particular about it, I can only guess from what has already been established.  That is: It was a “one off” built on a Fiat chassis (a 510, I think).

Partly correct. The car is indeed made by "Officine Meccaniche Achille Andreoli & Figli" in Codigoro, Ferrara.

However, based on my information, there are no relations with OM, neither Andreoli was a car dealer.
About the year, it's not really clear even to me, so let's assume 1922 is correct.

To get the point, I still need to know the base and the name.

Not that there is a specific story behind the base-model, and this is the most interesting aspect of this vehicle.
And it also has a name (it's on one of its advertisings).

The base chassis turns out to be easy.  It's a Fiat 15ter truck chassis.  It's interesting that they are trying to make a luxury car out of a truck chassis. 

The name is more difficult.  Most places it is referred to a Car (Vittura) 25-35 HP, or Luxury Car (Lusso Vettura) 25-35 HP.  However I did find one ad that shows a four letter logo (with some small print underneath that I can't make out) so I'll guess that the logo is the name that you are looking for.  That makes the puzzle car an: AFEA 25-35 HP.
