OK. This is my first and probably only attempt at putting up a puzzle for you to solve.  I came across this recently online and I gather that the design received some notoriety in certain circles.  Do you know what it is?

As your "first and probably only attempt at putting up a puzzle", it would be a pity to delete it.
Let us know what thread it was and one of the editors will be happy to merge them, as usually done.
